There are several options. Nose surgery is performed by either an endo-nasal (all incisions are inside) or open (a small incision is done on the outside) technique. There is no one right way to do rhinoplasty, so do not listen to a physician who tells you that there is only one way to do nose surgery. Your physician should be able to perform a nose job using both the open and endo-nasal approach.
The open technique is used primarily for patients who have thick skin and want a more refined tip. This technique is also used for patients who have had multiple surgeries with less than satisfactory results and need a redo or a reconstruction for an airway obstruction. It can be used to repair noses that have had an excess amount of cartilage removed during their first surgery and need additional cartilage placed into the nose. The endo-nasal or closed technique is used for patients who have bumps and need very little tip work. This technique can also be used to work on the tip.
The single most important technique is picking the right surgeon to do your nose surgery. Do your homework. Nose surgery is one of the most difficult cosmetic surgeries to get right. It takes hundreds of surgeries to understand what you are doing as a surgeon and many years of experience. You should pick a physician who is trained in facial plastic surgery and plastic surgery and who specializes in nasal surgery. Unlike surgeries in other parts of the body, it's hard to hide a bad nose job.
